Respecting your Data Privacy, we at EACMed are committed to handling with care your data and our obligation to you with respect on how we collect and manage your Personal Information. This aims to ensure that all appropriate standards for Personal Information protection in compliance with Republic Act No. 10173 (or the Data Privacy Act of 2012 or DPA), its implementing rules and regulations and other applicable and related laws and regulations, including the issuances of the National Privacy Commission (NPC), collectively, the Privacy Laws, are put in place and implemented efficiently and effectively.

The statement below describes how your Personal Information are collected, stored and processed.

1. WHAT PERSONAL INFORMATION WE COLLECT FROM YOU. With your consent, we collect and gather data in many ways, including but not limited to:

a. information which may be used for identification purposes (e.g. name, addresses, date of birth, contact numbers);

b. your Personal Information also includes Sensitive Personal Information, which refers to your race, ethnic origin, marital status, age, color, religion, health or medical background, Government issued ID’s;

c. contact information of your next of kin, relatives or guardians;

d. current and past medications, medical history, including when relevant, a family medical history and previous admissions;

e. results of laboratory tests, x-rays, scans and other diagnostic examinations;

f. other information established by law to be kept classified, which may be necessary to assist us in providing you with a better health care.

2. HOW AND WHY WE COLLECT YOUR PERSONAL INFORMATION. Your Personal Information are provided by you through interviews and document forms which you or a guardian fills out. We use your Personal Information for the following purposes:

a. to help us better understand your current health status and for reference on continuity of care should you return to us for follow-up checkup or confinement;

b. to liaise with your HMO or Company who you were affiliated with;

c. for processing of your benefits and payment on your bills;

d. for research; e. for submission to Government Institutions like PhilHealth and DOH.

3. DO WE SHARE YOUR PERSONAL INFORMATION. Yes, there will be instances that we share you Personal Information to Government Agencies which lawfully collects data like the Department of Health (DOH) which requires submission of relevant Personal Information of Patients with certain non-communicable and/or communicable disease for purpose of disease surveillance and monitoring; and the Philippine Health Insurance Corporation (PhilHealth) so we can process claims deducted from your bills.

We may also share Personal Information to fulfill legal mandates like when ordered lawfully by a court or a Government Agency.

We may also share your Personal Information with HMOs and/or Companies with whom you are affiliated with, and with whom you consented to the sharing thereof.

4. HOW DO WE SECURE YOUR PERSONAL INFORMATION. In our commitment in ensuring the security of Personal Information, our data is stored in a manner that reasonably protects it from misuse and loss, unauthorized access, modification and disclosure. Only authorized personnel have access to your Personal Information at all times.

We store your Personal Information in accordance with the Department of Health (DOH) retention guidelines.

5. WHAT ARE YOUR RIGHTS AS A DATA SUBJECT. Under the Data Privacy Act of 2012, you have the following rights:

a. right to be informed;

b. right to access;

c. right to object;

d. right to erasure or blocking;

e. right to damages;

f. right to file a complaint;

g. right to rectify; and

h. right to data portability.

6. CHANGES TO THIS PRIVACY NOTICE. From time to time, we may change or update our Privacy Statement, Privacy Policy and practices to comply with the Government and regulatory requirements, to adapt to new technologies and protocols, to align with industry practices or for other legitimate purposes.
